Please note: Effective February 17th, 2023, we are no longer approving prescriptions for online pharmacies, such as Chewy and 1800 PetMeds. Clients are welcome to purchase their medications from the pharmacy of their choosing, but clients will need to pick up a written prescription from the hospital.

Please Note: We will be closing early at 12.30 PM on Wednesday, November 27 and will be fully closed on Thursday, November 28th in observance of Thanksgiving!

International Health Certificate Information

Los Feliz Small Animal Hospital is happy to help you get an International Travel Certificate. When traveling with your pet(s), there may be animal health requirements specific for that destination. Let us assist you with the pet travel process. Please fill out the form below as completely and accurately as possible before your appointment. If you have any questions about this form, feel free to contact us at 323-664-3309.

Please note that it is the responsibility of the pet owner to make sure their pet has met the requirements of the destination country and that failure to meet the requirements may result in problems gaining certificate endorsement or difficulties upon arrival in the destination country.
